  • Schedule G – Executory Contracts and Unexpired Leases
  • Schedule H – Your Co-debtors
  • Schedule I – Your Income
  • Schedule J – You Expenses
  • Declaration About An Individual Debtor’s Schedules
  • Your Statement of Financial Affairs
  • Chapter 13 Statement of Your Current Monthly Income and Calculation of Commitment Period
  • Calculation of your Disposable Income
  • Mailing matrix
  • Your Statement About Your Social Security Numbers

  • Chapter 13 Plan. The Model Plan must be used and is available at http://www.flmb.uscourts.gov/procedures/documents/Chapter13Plan.pdf
  • A credit counseling certificate
  • Appropriate Filing Fee in the form of a money order, cashier’s check or exact cash. See Fees
